Space is limited to 28 teams for entry into the event.

Sponsorship Payments
Any sponsor who has not paid by the deadline will not be recognized on any banners. Must be paid in full by deadline date. If not paid in full by the deadline, the spot will be given away!

• White and Blue Sponsors: Fill out the application for 4 Golfers
• Individual Player
• Stars + Stripes, Purple Heart or Whole Sponsors: You Do Not need to fill out the application. These sponsorship levels do not include golf.
